Abstract:

Survey data from our mobile body-scanning user base indicates that a substantial fraction of users is undergoing weight loss assisted by GLP-1 or similar medications. Longitudinal weight trajectories observed in our database corroborate this through their characteristic front-loaded loss pattern, matching what has been reported in pivotal GLP-1 trials. This real-world context motivates a careful look at how body composition predictions behave when applied longitudinally to individuals undergoing substantial weight loss. We have assembled a large longitudinal dataset spanning a wide range of weight-loss magnitudes, rates, and baseline body habitus. We characterize how the fraction of total weight change attributable to lean mass varies with magnitude, rate, and baseline BMI, and compare these patterns against expectations from established cross-sectional reference formulas applied longitudinally. We additionally examine other body composition metrics as well as anthropometric measurements along the same trajectory dimensions. Unlike the body composition metrics, which come from regressions trained on cross-sectional data, the anthropometric measurements are extracted directly from segmented images captured during the mobile scan process and therefore should naturally track longitudinal change. Additionally, they can help identify which specific measurements drive any trajectory-dependent patterns observed in the body composition metrics. Alternative model formulations augmenting the standard cross-sectional inputs with weight-loss magnitude and rate are evaluated to surface testable hypotheses. These hypotheses are tested against a separate small-sample longitudinal DXA-paired study currently in progress at Pennington Biomedical Research Center in which participants are receiving GLP-1 therapy for weight loss, with the objective of determining whether trajectory information leading up to a scan can systematically improve body composition estimation in real-world weight-loss populations.
